Facilities Request Form

We aim for 1-2 days response for all submitted requests; however, submitted requests are completed on a priority basis. This form is for general maintenance, custodial requests, and event planning/set-up (NOT emergencies). Please fill out this form completely, or your request may be delayed. You will receive confirmation of receipt, and Maintenance/Custodial staff will follow up for scheduling. Confirm all emergencies (Alarms – Fire – Spills – Water) first to the maintenance staff at 605-367-5289 or 605-929-2823.
