Please submit this form for each course(s) and term(s) you are teaching. Books/materials are not copied over from one term to the next.

Fall 2016 Attendance Confirmation is available in Webadvisor in late July. All students who have registered for Fall 2016 classes are required to complete the online attendance confirmation form. This must be submitted before 9/2/16.
